Please register on December 12th at the Radisson SAS Scandinavia Hotel. The Registration Desk opens at 8:00AM, and is located directly outside meeting room - SWEDEN. Acceptable forms of payment are credit cards and checks (made payable to INT Media Group) One Day Seminar Price (Wednesday December 12th) - $895.00 USD. (7,300.00 DKK) If you are planning to pay by check: You will need to print this registration form, complete it and mail it with your payment. You may also fax it WITH CREDIT CARD PAYMENT to (203) 286-1010 - ATT: Allison Alessio Purchase Orders are acceptable, however, pre-payment is required to register. Cancellations and Substitutions
If you must cancel for any reason, we must receive written notice of cancellation by November 12, 2001. Your registration will be refunded less a U.S.$100.00 (800.00 DKK) processing fee. Cancellations after November 12, 2001 are non-refundable. You may transfer your registration to another person at any time by providing written authorization.
