If you are paying by check:
You will need to print this form, complete it and mail it with your payment. Payment by mail must be received NO LATER than May 23, 2007. If payment is not received by May 23, 2007, you will need to register on-site. You will not be registered for the conference until payment is received.

Cancellations and Substitutions
If you must cancel for any reason, notify our registration department in writing by fax +1 203 286 1010 or email registration@incisivemedia.com by April 27, 2007. Your registration will be refunded less a €100 processing fee. Cancellations after April 27, 2007 are non-refundable. You may transfer your registration to another person at any time by providing written authorization.
